    Church Gear Pre-Order Form

    Church T-Shirts range from sizes Child Small to 5X. T-shirts are $12 per shirt. T-shirt sizes starting at 2X require an additional $2 per X. For example a 2X shirt is $14. Hats are $25 per hat. Hats are one size fit only. The hats are blue denim and black denim.
